Local Community Champions Can Make Or Break On-field Health Initiatives. In The Development Sector You Can Plan, Raise Funds, Arrange Adequate Resources And Still Fail To Deliver. Many Of The Initiatives Face These Consequences And The Reason For The Failure Or The Success Simply Lies In Being Persistent. In July Of 2018 Liver Foundation West Bengal (LFWB) Focused On Creating A ‘hepatitis Free Village”. The Concept Was Novel. LFWB Will Select A Remote Tribal Village, Test All The Villagers For Chronic Viral Hepatitis, Bring The Hepatitis Cases Under Treatment And Immunize The Rest Of The Villagers. The Villagers Will Not Have To Spend A Single Penny And The Whole Initiative Will Be Conducted In Their Village. The First Selected Village Was Sitarampur, A Village Of “Ghatwal” Community, On Top Of Ajodhya Hills In Purulia District Of West Bengal With A Population Of 500 Residents And The Nearest Health Center Being 6 Km Away From The Village. On The Day Of The Program Launch The Courtyard Was Full. However, Only One Out Of Three Villagers Got Screened And Eventually Only Half Of Screened Individuals Got Fully Immunized. This Was A Major Setback. The Program Required Multiple Trips To The Remote Area For Over 7 Months. There Was No Guarantee That Repeating The Program In The Village Would Ensure Full Immunization. Looking Into The Reasons For People Dropping Out Of Immunization, It Was Found That People Were Not Conscious About The Health Benefits Of Hepatitis Immunization And Prioritized Their Regular Chores Over Getting Immunized On Schedule. In Addition To The Single Big Awareness Campaign, LFWB Concentrated On Small, But Regular Whispering Campaigns. It Involved Three High School Students Of The Village, A Few Women Of The Village Who Had Completed Immunization, A Person Whose Hepatitis Was Detected In The First Camp And Was Going Through Treatment, As Well As, One Of The School Coordinators As Local Advocate. These Advocates Continued Regular Awareness Campaigns Regarding The Hepatitis Testing And Immunization In The Village And Mobilized Their Neighbors And Acquaintances To Testing And Immunization Camps. The Second Phase Reported A Lower Dropout As 80% Of The Participants Were Fully Vaccinated. 4 Of The Villagers, Who Could Not Be Available For Final Dose At The Village, Got Themselves Vaccinated At Their Location. The Lessons Learned Were Repeated In The Katabuni Village Of Birbhum. This Tribal Village Also Marked 80% Complete Immunization. This Evidence Indicates The Critical Role Of Continuous Community Owned Awareness Campaigns For The Success Of Health Initiatives Of Medium Duration. More Evidence Needs To Be Generated On How Community Involvement Affects The Success Of Prolonged Health Initiatives And What Components Of Community Participation Can Be Optimized For The Best Outcome.

Over Time, Conditions That Damage The Liver Can Lead To Scarring (cirrhosis), Which Can Lead To Liver Failure, A Life-threatening Condition. But Early Treatment May Give The Liver Time To Heal. In 2015, The Total Number Of Liver Disease Registered In India Is Noteworthy Since It Alone Contributed To 18.3% Of The Two Million Overall Liver Disease–related Deaths. In Addition, Chronic Liver Disease Is The Cause Of Premature Death And Disability In India. However, We Have A Lack Of Health Policy Related Data That Hampers Building Effective Planning. With The Intention Of Addressing The Issue Regarding Chronic Liver Diseases We Need An Effective Interventional Strategy. In This Present Study, We Selected Eleven Hospitals From Various Zones Of India To Understand The Current Situation Of Chronic Liver Disease In India. This Study Reported That Viral Hepatitis Is The Leading Cause Of Chronic Or Long Term Liver Disease. On The Other Hand, Alcohol Has Been Considered As The Major Cause Of Cirrhosis. The Study Explained That There Are Some Inter Regional Differences Between Different Parts. Hepatitis C Was Found Mostly In North India, While Hepatitis B Was Found In The East And South Parts Of India. It Was Found Alcohol Consumption Is The Major Cause Of Chronic Liver Disease In The North-east, At The Same Time As Non-alcoholic Fatty Liver Disease Is The Well-known Cause Of Long Term Liver Disease In The West Part Of India. On The Other Hand, Viral Hepatitis Is The Major Cause Of Liver Cancer. CLD Is Gradually Becoming A Public Health Priority In India. We Found Two Important Aspects Like Alcoholism And Diabetes. India Needs A Liver Disease Registry And Morbidity/mortality Database To Fight The Upcoming Trouble Of CLD In The Country. This Study Is Helpful To Understand The Current Situation And Based On That We Can Plan For The Future.
